The Mushroom Kingdom has been hit by a virus, and it's up to Dr. Mario to wipe it out. Line up the falling pills to create a line of four and eliminate the viruses. When all the viruses are cleared, you move onto the next round.
Released alongside the NES version, the Game Boy release features the same gameplay and game modes. Due to the hardware's limitations, the viruses appear as black, white and gray, and the 2P Mode can only be played using a Game Boy Link Cable and two copies of the game.
